This study examined the association between perioperative glycemic control, measured by hemoglobin A1C levels, and adverse outcomes following arthroscopic rotator cuff repair in diabetic patients. Analyzing over 3,700 cases, researchers found that patients with A1C levels above 8% had a significantly higher risk of deep postoperative infection (1.14% vs. <0.3% for A1C <6%). No increased risk was seen for revision surgery or postoperative stiffness. However, the predictive accuracy of A1C >8% for infection was limited (AUC=0.64), suggesting that while elevated A1C is a risk factor, other factors also influence infection risk and should be considered clinically.
