This study examined 149 contact athletes undergoing arthroscopic posterior capsular labral repair for unidirectional posterior shoulder instability, with an average 13-year follow-up. The revision surgery rate was 5.4%. Key findings showed that patients needing revision had significantly worse pre-operative stability and lower ASES scores. A smaller glenoid bone width was the sole significant MRI risk factor for recurrence, while age, gender, sport level, and glenoid version were not. Return-to-play rates were significantly lower in the revision group (50% overall, 17% at same level) versus non-revision athletes (94% overall, 72% same level).
