This prospective, double-blinded randomized study compared arthroscopic Bankart repair with and without remplissage in patients with anterior shoulder instability and Hill-Sachs defects. Over six years, 108 patients were randomized across two centers. Both groups showed significant improvement in WOSI, ASES, and SST scores, with no differences in range of motion, pain, or return to sports at two years. The remplissage group had a strong, though not statistically significant, trend toward fewer re-dislocations. MRI showed high rates of defect healing with remplissage. Longer follow-up is needed as dislocations occurred post-study. Overall, remplissage appears safe without compromising function.
