This study examined meniscal tears and extrusion in middle-aged patients without or with mild arthritis, using long-term MRI and clinical follow-up. Meniscal tears, especially posterior horn horizontal cleavage types, are common and linked to accelerated joint space narrowing. Medial extrusion is more frequent, but lateral extrusion strongly predicts faster progression to total knee arthroplasty. Lateral meniscus integrity is crucial for load distribution. While tears accelerate arthritis, extrusion—particularly lateral—poses a greater risk. The findings highlight the need to explore repair strategies for horizontal cleavage tears and consider early intervention for symptomatic lateral meniscus extrusion to slow osteoarthritis progression.
