This study evaluated outcomes of primary ACL repairs with suture augmentation for proximal femoral wall avulsion tears (Sherman type 1 and 2) in 28 patients. Over nearly three years of follow-up, 4 patients required revision ACL reconstruction, showing a low failure rate. Significant improvements were seen in physical function (KOS, VR12 physical), pain, SANE, and WOMAC scores postoperatively. Mental health and activity scores showed non-significant improvement. The technique involved microfracture and suspensory fixation with internal bracing. Authors conclude that, with strict indications, primary repair with augmentation is effective, improves patient outcomes, and preserves options for future reconstructions.
