This study analyzed active duty service members undergoing primary MPFL reconstruction for patellar dislocation to assess return to full duty and impact activities. Of 103 patients, 49.5% returned to unrestricted activity, while 26% had medical separation. Recurrent or bilateral instability and preexisting osteoarthritis significantly reduced the likelihood of return to duty. Revision surgery rates were low (2.9%). Age and gender did not influence outcomes. Limitations include retrospective design, varied surgical techniques, and database inconsistencies. Future research will explore military-specific factors affecting retention. The findings highlight the need for improved treatments enabling service members to resume high-level physical activity and active duty status.
