This study evaluated intraoperative fluoroscopy's role in confirming femoral cortical suspensory fixation placement during ACL reconstruction. Historically, bone-tendon-bone grafts with interference screws have been standard but carry complications like donor site morbidity and anterior knee pain. All-inside techniques with cortical suspensory fixation are less invasive alternatives. In a retrospective review of 100 patients, those with intraoperative fluoroscopy (60 patients) had significantly more optimal device positioning compared to those without it (40 patients), reducing suboptimal placement from 10%. While limited by retrospective design and unequal group sizes, results suggest fluoroscopic imaging helps ensure accurate device placement and may improve surgical outcomes.
