Video Summary
Ramp lesions are tears at the medial meniscocapsular junction, commonly associated with ACL injuries (reported prevalence ~10–40%+). They’re frequently missed unless the surgeon deliberately inspects the posteromedial compartment, ideally with a 70° arthroscope, since a 30° scope may not visualize the lesion well. Predictors include posterior medial tibial bone marrow edema on MRI and striations on the medial femoral condyle. Repair options include all-inside meniscal devices or a preferred technique using a single posteromedial portal with a cannula and suture-passing/lasso device. Portal placement should be more proximal than expected to access around the condyle. Typically 1–3 sutures (often two) are used.
