Video Summary
The video demonstrates a lower trapezius tendon transfer with arthroscopic assistance for an irreparable posterosuperior rotator cuff tear in a 52-year-old patient. In beach-chair position, the lower trapezius is detached from the scapular spine through a transverse incision. An Achilles tendon allograft is prepared with strong nonabsorbable sutures. Arthroscopically, the greater tuberosity footprint is prepared and microfractured, and a passage is created between the infraspinatus and posterior deltoid for graft transfer. The graft is fixed to the humerus using anchors, then connected to the lower trapezius with a Pulvertaft weave. The wound is closed and the arm immobilized in abduction and slight external rotation.
