Video Summary
In this 2023 lecture on hip preservation, Dr. discusses when open hip surgery is necessary versus hip arthroscopy. Open surgery, such as periacetabular osteotomy (PAO), is clearly indicated in frank hip dysplasia and adult sequelae of Perthes disease. Borderline dysplasia (lateral center edge angle 18-25°) presents challenges; hip arthroscopy can work if meticulous soft tissue repair occurs, but PAO may be better for instability or poor coverage. Femoral version abnormalities and severe deformities may also warrant open procedures like femoral derotational osteotomy or surgical hip dislocation. Total hip replacement is advised for advanced arthritis. Accurate imaging and clinical evaluation are essential for appropriate treatment choice.
