Pdf Summary
The webinar "Elbow Treatments: Throwing Injuries, PLRI, Valgus Extension Overload and More" (4/13/2021) presented by Dr. Eric I. Ferkel and faculty Drs. Reza Omid, Surena Namdari, and Grant E. Garrigues covered a range of topics focused on complex elbow injuries, their diagnosis, surgical treatment, and rehabilitation strategies.

Dr. Namdari discussed posterolateral rotatory instability (PLRI) and lateral elbow instability, emphasizing the anatomy of the lateral collateral ligament complex and five keys to surgical success: accurate diagnosis, precise ulnar and humeral tunnel placement, graft choice (often palmaris longus), and rehabilitation protocols. Common causes of PLRI include trauma, chronic conditions, overuse, and iatrogenic factors. Diagnosis involves clinical signs like pain and mechanical symptoms, supported by imaging (X-ray, MRI). Rehabilitation typically entails initial immobilization followed by graduated motion and strengthening.

Dr. Omid highlighted distal biceps and triceps tendon injuries, detailing anatomy, pathophysiology, clinical evaluation, and management. Distal biceps ruptures often result from eccentric contraction, leading to weakness in supination and flexion. Surgical repair options include single versus double incision techniques, with no significant outcome differences but varied complication profiles. Heterotopic ossification (HO) is a common complication after repair, mitigated by indomethacin prophylaxis. Triceps tendon ruptures are rare, often requiring surgical repair when complete. Physical exam maneuvers like O'Driscoll’s Hook Test assist diagnosis.

Dr. Garrigues covered ulnar collateral ligament (UCL) injuries related to valgus extension overload, common in throwing athletes, reviewing risk factors, outcomes, and surgical techniques.

The session also included case discussions and literature references supporting current best practices. Attendees were informed about AANA membership benefits and educational resources for ongoing learning in elbow injury management. Overall, the webinar provided comprehensive, evidence-based insights into diagnosis, surgical techniques, and rehabilitation for complex elbow pathologies affecting athletes and active patients.
