Anand Sethanath presented a study comparing isolated bone-patellar tendon-bone (BTB) autograft ACL reconstructions to combined ACL and anterolateral ligament (ALL) reconstructions using hamstring tendons. Analyzing 2,000 matched patients over 15 years, results showed combined procedures had a threefold lower graft failure risk, including in high-risk younger patients. Combined reconstruction also reduced re-operations related to graft rupture, arthrofibrosis, and secondary meniscectomy. These findings align with other studies favoring combined reconstruction for improved graft durability and fewer complications. The study supports individualizing ACL graft choice but highlights benefits of adding lateral extra-articular procedures for better outcomes.
