This study evaluates distal tibial autograft (DTA) reconstruction using a novel double suture-button fixation for significant glenoid bone loss. Compared to traditional iliac crest autografts or screw fixation, DTA offers anatomic restoration with less donor morbidity and improved fixation. In 29 patients, the arthroscopic procedure showed no dislocations at 10 months, and 89% graft healing on CT. Clinical outcomes (WOSI 82.4, ASES 94.8) were favorable, with minimal complications. Results suggest that double suture-button fixation is as effective as screw fixation for DTA, providing a viable, anatomically congruent, minimally invasive graft reconstruction method.
