Video Summary
For AC joint surgery, avoid operating on all cases, as non-operative treatment often yields adequate results, especially for type 3 and 5 injuries. Late reconstructions are effective if needed later. If surgery is chosen, perform an anatomic reconstruction of both conoid and trapezoid ligaments using a graft, and preserve the distal clavicle for stability. Ensure the coracoid tunnel is centrally placed using the smallest drill to prevent fractures. Use fluoroscopy for accurate reduction confirmation due to anatomical variation. Postoperative rehab should be slow, with immobilization and limited forward flexion for 4-6 weeks to prevent failure.
