This study evaluated outcomes of arthroscopic Bankart repair in adolescents with anterior shoulder instability, focusing on bone loss factors predicting failure. Among 59 patients (median age 16), 31% experienced recurrent instability or required revision. While off-track lesions were rare (8.5%) and not predictive of failure, larger Hill-Sachs defect measurements (interval and depth) were linked to higher revision rates. Patient-reported outcomes were worse in those with recurrent instability. The authors suggest considering additional procedures, like remplissage or glenoid augmentation, not only for off-track lesions but also for on-track lesions with significant Hill-Sachs defects, to reduce failure risk in adolescents.
