This talk discusses the evolution of ACL surgery, focusing on a novel biologic approach called Bridge-Enhanced ACL Restoration (BARE). Unlike traditional graft-only methods, BARE uses a scaffold infused with blood and growth factors to create a healing environment promoting natural ACL tissue regeneration. Animal studies showed improved ligament healing and less cartilage damage. Early human trials, including adolescents, demonstrated safety, similar clinical outcomes, preserved hamstring strength, and comparable re-tear rates to standard reconstruction. BARE restores ACL anatomy more closely and supports earlier recovery. Ongoing studies aim to compare BARE to patellar tendon grafts and assess long-term osteoarthritis prevention.
