This presentation reviews ACL reconstruction in pediatric patients with open growth plates (FICEs), focusing on age-based surgical techniques. Younger children (prepubescent) typically receive FICE-sparing IT band techniques, favored for low growth disturbance and good outcomes. Pubescent adolescents with some growth remaining get trans-FICE tunnels with hamstring or quadriceps grafts. Older teens use adult-type reconstructions. The IT band method, despite being deemed non-anatomic, shows excellent functional outcomes and low complications. All-epiphyseal techniques offer anatomic advantages but higher growth disturbance risk and limited age applicability. The PLUTO multicenter study shows similar retear rates across techniques, with lower complications for IT band surgery.
