The Mars group investigated surgical factors influencing outcomes in revision anterior cruciate ligament (ACL) reconstruction. Enrolling 1,234 patients since 2006, with 77% follow-up at six years, the study analyzed approaches, tunnel positions, fixation methods, and biological enhancements. Results showed that anteromedial portal approach improved pain and function scores, while using interference screws for fixation led to better outcomes. Unexpectedly, less optimal tunnels sometimes had improved results, and biological enhancements had no impact. Secondary factors like higher baseline scores, non-smoking, and lower BMI improved outcomes. Findings aid surgeons in optimizing revision ACL surgeries and highlight the need for further research on causal effects.
