This study evaluated labral augmentation using iliotibial band autografts or allografts in revision hip surgeries with inadequate labral tissue. In 88 patients (mean age 32, 5-year follow-up), augmentation improved patient-reported outcomes significantly, with 93% survivorship (defined by avoidance of total hip arthroplasty). However, 14% required revision arthroscopy, typically due to adhesions. A secondary analysis suggested allografts had a higher revision rate from adhesions than autografts, influencing a shift toward autografts. Despite limitations like heterogeneous pathologies and interventions, labral augmentation appears a valuable preservation strategy in revision hip surgery, though fibrosis management remains a clinical focus.
